New Patient?

If you’re new to West Georgia Eye Care Center, save yourself time and energy by filling out your paperwork before your appointment. Just click on the New Patient Form (below). The forms are in PDF format, so you can easily print them out and complete them by hand or download them to your computer. Once finished, bring the printed paperwork into the office on the day of your exam.

Appointment Cancellation & No-show Policy

Our goal at West Georgia Eye Care Center is to provide our community with excellent and timely care. Late cancellations and “no-shows” restrict access to individuals requiring medical attention; therefore, we have implemented the following policy so we can better utilize available appointments for patients in need of care.  We thank you for trusting our team with your eye care needs and appreciate your cooperation in helping us to best care for our community.

If you need to cancel or reschedule an appointment, we ask that you notify our office as soon as possible.  We do have many patients waiting, and if you give us more than 24 hours’ notice we will be able to use your appointment time to serve someone else.

Effective May 1, 2021

  • A patient who does not show for two scheduled appointments in any one year (including failing to give 24-hour advance notice before canceling or rescheduling) will be charged a $25.00 fee prior to scheduling any further appointments.
  • The fee is charged to the patient, not the insurance company. It is the PATIENT’s responsibility to pay the No Show fee.
  • As a courtesy, West Georgia Eye Care Center will send appointment reminders to the phone number and/or email address on file for patients. An appointment reminder will be sent by email/ text up to 30 days prior to the appointment, 14 days prior to the appointment an email or test will be sent with an option to confirm or cancel your appointment and a final reminder will be sent by call or text 3 days prior to your appointment.
  • We understand there may be times when an unforeseen event occurs and you may not be able to keep your scheduled appointment. If you should experience extenuating circumstances or need to discuss your situation, please call to speak with our Office Manager for assistance.

Patients who need to cancel or reschedule an appointment can do so by calling (706) 323-3491. If calling after hours, please leave a message with your name, date of birth, and phone number.
